This patient didn’t like the discoloration and how bulky this filling felt. Within 45 minutes we were able to take out the old filling, to remove decay found beneath the old restoration, and to restore with a bpa-free esthetic filling that should last decades. I would prefer to do this rather than do a porcelain crown or veneer. This procedure is more conservative, quicker to do, and a lot less expensive. The patient was more than happy with the result.

There was a lot going on in this case we did this morning.
Patient wanted to repair a chipped tooth.
The history here is this tooth has been worked on twice before.
Do you simply add composite to what's there or do you remove all of the previous filling?
Do you numb the tooth? If you do, what anesthetic do you use and where do you insert the needle?
Do you add length to match the other tooth? Or do you take all the teeth into consideration?
Do you go for an ideal shape of tooth or a natural-looking (not too perfect) look?
Do you use multiple shades of composite to match the other tooth? Or will one shade work?
Should you do the same to the other front tooth? Or why not all 6 front teeth? What about 8? 10? 12?
Do you remove more tooth to get a thicker layer of composite to cover the darker tooth or to increase durability?
What filling or bonding material is least reactive, most esthetic, or most durable?
So many questions require a lot of decisions.
Do you believe that all filling procedures are the same?
Do you believe that all dentists would know to ask these difficult questions?
Do you believe every dentist can produce equal results?
This case may have taken me less than an hour to do, but it took me thousands of hours to know what, how, and why to do it.

If you have stains around old fillings, sometimes you can have the fillings refined and polished. In this case it only took about 2 minutes of my time. And since I was doing other treatment in this patient's mouth, I went ahead and did this for her.

Here's another example of what can be done in 90 minutes.
The 12 year old girl didn't like the color of her front teeth. 6 years ago she chipped her front tooth. And since then the filling has chipped a couple more times.
She plays soccer and is fearful that the filling would continue to chip.
Another dentist recommended she get porcelain veneers when she becomes an adult.
After discussing some treatment options , I recommended we try to fix the color, shape, and length of her front two teeth while still conserving her tooth structure.
As you can see, her original tooth fracture was significant.
Using laser dentistry I was able to bond a nice ceramic like filling to her existing teeth. I added length to make it more natural (central incisors should be longer than their neighboring lateral incisors). Because of prestine bonding techniques, I am confident the filling would be durable enough to withstand most blows.
When she comes in next, I will use a tool to shape the fillings further. I think the right filling is a little wide but nothing 2 minutes of recontouring can't fix.
20 yrs ago, cosmetic dentistry was my first love. Now, I am enjoying it once again, only with less toxic materials and more conservative techniques. (Let's not forget that this procedure takes half the time and half the costs of porcelain veneers)

Chief Complaint:. Patient's husband wants her teeth to look better.
The Challenge: 1. Remove as little tooth as possible 2. Improve esthetics 3. Do not interfere with patient's existing bite pattern 4. Use healthier dental materials 5. Accomplished in a timely manner 6. Last a long time
The Result: Using a dental laser, I removed the old composite fillings, prepared the tooth without numbing the area, and treated the enamel and dentin for optimal bonding of a ceramic-based filling material. In less than 3 hours (2 separate visits) and less than the cost of 2 porcelain veneers or crowns, this patient and her husband were extremely happy with the results.
I expect the fillings to last for more than a decade, possibly forever.
Unfortunately, modern dentistry is about doing higher dollar (and more invasive) procedures like veneers and crowns.

What can happen to someone in 2 hours? How about transform someone's smile?
Could I have used a layering technique to help match the color of the teeth? Yup.
Could I have used a material that polishes better? Yes.
Could I have shaped the teeth to look less jagged? Of course.
But when it is all said and done, the resulting smile on this patient's face said it all.
By the way, I would guess that 95% of dentists would not have even offered this type of procedure (fillings).
They would have instead recommended 3-4 crowns. And if crowns were recommended then build-ups and root canals and possibly posts would have been recommended also. The costs for such procedures can be upwards of 2 to 4,000 dollars. Per tooth!!!
This patient paid less than that for a more conservative (and in my opinion, better) restoration.

Just wanted to share what we do here.
Holistic, conservative dentistry can be cheaper and *may* be better for some of you, but it is also more time-consuming, and could require more skills and the use of additional technologies.
CASE STUDY:
This 30 yr old man from St. George, Utah had a tooth that has bothered him off and on for 2 years. Several other dentists recommended a root canal and a crown (total costs varies per dentist but it can range between $1800 to $4000). After our xray evaluation and examination we determined that we can attempt to save the tooth with a conservative biological filling. In one visit we used ozone gas, lasers, biocompatible anesthetic, BPA-free filling materials and of course, techniques to do the best dentistry for his tooth and body.
A day later, the tooth stopped bothering him and patient was so happy to have saved the tooth (without doing a root canal and crown). If comparing apples to apples, our fillings cost more than "their" fillings. But that's not what needs to be compared. We saved this man's tooth by doing more conservative dentistry. The correct comparison is between our filling vs their root canal and crown. The cost difference is substantial. He paid about 25% of what he would have paid at any of the other dentists.
Before automatically saying you can't afford us, remember that it is not about comparing costs per treatment, it is about getting the best dentistry that is healthier and more predictable. And usually this type of dentistry is more cost effective in the short AND long term.
But that's not the end of the story... This patient went home and got a cleaning at his local dentist. That dentist mentioned that the filling was done well but she then, recommended a crown. 🙄 Beware of the business of dentistry.

As the saying goes, we learn more from our mistakes than our successes.

A: Remove the TOXINS that are constantly triggering it to go haywire.
Always find the root cause and ask where those toxins are coming from? Did you eat them for lunch? Did you breathe it in while sleeping? Or have they been in your mouth this whole time?
So as you plan your To-Do list for when this lockdown ends, please do not forget to take care of your health and do not forget to see your biological dentist.

In general, it is not safe to go to see the dentist during a pandemic where the virus is airborne and affects one's upper respiratory tract like coronaviruses.
